Supervisor's Evaluation and Recommendation

An applicant to the doctoral program in counselor education and supervision at Duquesne University has identified you as a professional reference who has supervised or is very familiar with the quality of his/her counseling and other professional activities. Please evaluate this applicant to the Duquesne University Doctoral Program in Counselor Education and Supervision on the basis of his/her skills as a counselor and effectiveness as a professional.
