Pre-Clinical Health Requirements Form - Complete required exam, lab tests and titres prior to the start of the Fall semester. Bring completed forms to UPNS 108 Freshman Seminar course for further instructions.
Clincal Requirements*
Freshman students will complete these requirements during the Fall semester as part of the UPNS 108 Freshman Seminar course. All requirements must be completed prio to the start of the freshman Spring semester.
