Immunization Policies and Forms

New students must complete health history and immunization forms. These must be completed prior to registration for classes. Download, print out and complete. You may need to review these with your doctor.

Some Nursing, Pharmacy and Health Science students may need to submit forms for pre-clinical requirements.

Stay Healthy - Mentally, Physically and Emotionally

The Health Service is located in Duquesne Union, 2nd (Main) Floor.

  • Call 412.396.1650 with questions or to make an appointment.
  • Academic year hours: weekdays, 7:30 a.m. to 6 p.m. (close at 5 p.m. on Fridays)
  • Doctor in daily, nurse practitioners, PAs and RNs available all day.

Services include routine treatments for illness/injury and physicals, TB skin testing, allergy injections; wellness exams (checkups) for men and women and health education.

Counseling Center - Free and confidential counseling.
