Philanthropic and Service Hour Forms
To record philanthropic monies raised and record student service hours throughout the academic year, the DUV Office has provided forms to help student groups record their service hours.
Log onto Campus Link through DORI to printout the Philanthropic Form and/or the Service Hour Form, please return to the Center for Student Involvement, 305 Union to be recorded.
