| Imputed Income | IRS requires that the cost of employer sponsored term insurance in excess of $50,000 be treated as imputed income. This amount is shown on your pay stub and will be reported to the IRS on your W-2 form. Contact the Payroll Department at 412.396.6579 to determine the taxable value of your term life insurance over $50,000. |

Employee is responsible for applying for LTD benefits. LTD claim forms are available from the Disability Claims Manager.
