MEI Medical Evidence of Insurability
Required if amount is over $300,000 or increasing a level.
Imputed Income IRS requires that the cost of employer sponsored term insurance in excess of $50,000 be treated as imputed income. This amount is shown on your pay stub and will be reported to the IRS on your W-2 form. Contact the Payroll Department at 412.396.6579 to determine the taxable value of your term life insurance over $50,000.

Optional Dependent Life Insurance

Options Include $5,000 for spouse /$2,000 per child
$10,000 for spouse/$5,000 per child

Optional Accidental Death and Dismemberment

Selections $10,000 to $250,000
Employee Only or Employee/Spouse at 50% and Children at 15%
Benefits—Accidental Loss of life, both hands, feet, eyes 100% of coverage level
Loss of one hand, one foot, one eye 50% of coverage level

Long Term Disability

Long Term Disability replaces a portion of your income if illness or accident prevents you from working for an extended period of time.
Core 50% of base salary
Maximum of $2,000 per month
Premium is paid by Duquesne University
Buy Up Option 1 60% of base salary
Maximum of $5,000 per month
Buy Up Option 2 Lesser of 60% of base salary to $10,000 per month or
70% less deductible sources of income

Employee is responsible for applying for LTD benefits. LTD claim forms are available from the Disability Claims Manager.
