Updated and New Features for Duquesne's Benefits Program, Effective July 1, 2013.
ID Cards. Enrollment information is electronically sent to our providers after the open enrollment process is finalized. It usually takes 10 to 15 business days from the time each company receives the information to print and mail ID cards. You can also print temporary cards by creating your profile via the vendor's website. Dental and vision providers do not issue cards.
Cigna High Deductible Health Plan (Plan H) out-of-pocket maximums are decreasing. The single out-of-pocket maximum will decrease to $3,000, and the family out-of-pocket maximum will decrease to $6,000.
Emergency Room copayment will increase from $75 to $125. The copayment will continue to be waived if admitted. Employees are encouraged to use Urgent Care Facilities with a $25 copayment.
Elimination of $500 Medical Waiver.
Removal of GERD (Acid Reflux) medications from the Medication Therapy Management program.
Update in vision providers. The VBA (Vision Benefits of America) plan will end effective midnight, June 30, 2013. VSP (Vision Service Plan) will be the new vision provider with a choice between their Basic or Enhanced offerings. New in-network providers include Costco Optical and Visionworks. The plan permits services based upon the July 1 to June 30 plan year; it is not based on anniversary date. Current vision plan participants not completing the Open Enrollment process will be defaulted to the VSP Basic Plan.
Update in life insurance and long-term disability (LTD) provider and plan design. The SunLife plan will end effective midnight, June 30, 2013. MetLife will be the new insurance carrier for life insurance and long-term disability. Life insurance coverage over $50,000 is subject to imputed income as explained on page 22 of the Benefits Overview. Plan design changes include:
An increase in employer-paid core life insurance of one times base pay from $50,000 to $75,000 maximum.
A new employer-paid core accidental death and dismemberment (AD&D) policy of one times base pay to a $75,000 maximum (previous AD&D coverage was voluntary and employee paid).
An increase in maximum life insurance coverage from 5x salary with a limit of $500,000 to 5x salary with a limit of $750,000. Employees not completing Open Enrollment process will be defaulted to current optional coverage amount.
An increase in employer-paid core LTD maximum benefit from $2,000 per month to $5,000 per month.
Elimination of LTD Buy Up Option 2 due to the increase in LTD voluntary maximum benefit in Buy Up Option 1 from $5,000 to $10,000. Employees not completing the Open Enrollment process will be defaulted to core option.
Decrease in rates for employee life insurance and long-term disability.
Change in Optional Dependent Life selections to permit purchase for spouse and/or child(ren) with increase to maximum $30,000 for spouse and $10,000 per child. Rates will now be age-rated for a spouse. Employees not completing the Open Enrollment process will be defaulted to no Optional Dependent Life coverage.
Elimination of voluntary AD&D coverage.
